Adult Family Living (AFL) is a residential service that provides individuals with intellectual or developmental disabilities the opportunity to live in a supportive home environment where they receive assistance with daily living, community integration, and personal development.

TLC Support & Habitation LLC serves adults with intellectual and developmental disabilities, including individuals with autism and other behavioral health diagnoses. Our home is designed to support individuals who need structured care, supervision, and assistance with daily living.

We currently serve adults ages 18 and older, with a focus on young adults and adults who benefit from structured residential support.

Our services include:

· Assistance with Activities of Daily Living (ADLs)

· Medication administration and monitoring

· Community integration and participation

· Behavioral support and supervision

· Life skills development

· Meal preparation and nutrition support

· Transportation to appointments and community activities

· Health and safety monitoring

The Innovations Waiver is a North Carolina Medicaid program that provides services and supports to individuals with intellectual or developmental disabilities so they can live in community-based settings instead of institutional environments.

Yes. TLC Support & Habitation LLC works with individuals receiving services through the North Carolina Innovations Waiver and coordinates care with case managers and Managed Care Organizations (MCOs).

We coordinate services with several North Carolina MCOs, including:

· Trillium Health Resources

· Alliance Health

· Vaya Health

Referrals are typically made by:

· Care coordinators

· Case managers

· Managed Care Organizations (MCOs)

· Guardians or family members

Our home is structured to support individuals who require moderate to higher levels of supervision and assistance, including individuals with Level 3 and Level 4 service needs.

To maintain a family-style environment and provide individualized attention, we maintain a small residential setting with limited bed availability.
